基于代谢组学的难治性肾病综合征脾肾阳虚证尿液生物标志物筛选研究

         

摘要

目的 研究难治性肾病综合征 (RNS) 脾肾阳虚证患者的尿液代谢组学特征.方法 收集15例RNS脾肾阳虚证 (试验组) 患者和16例健康人 (对照组) 的尿液样本, 运用气相色谱-质谱 (GC-MS) 联用技术检测代谢图情况, 采用正交-偏最小二乘判别分析 (OPLS-DA) 进行模式判别和差异代谢物筛选.结果 2组尿液代谢物谱明显不同.与对照组相比, 试验组亮氨酸、缬氨酸、赖氨酸水平上调, 琥珀酸、马尿酸水平下调.结论 RNS脾肾阳虚证患者尿液代谢图谱发生改变, 发生改变的5种代谢物可能为该病证的生物标志物, 差异代谢物提示机体出现氨基酸代谢、三羧酸循环紊乱及肝肾功能异常.%Objective To study the characteristics of urinary metabolomics in patients with refractory nephrotic syndrome (RNS) with spleen and kidney yang deficiency syndrome. Methods Urine samples were collected from 15 patients with RNS spleen and kidney yang deficiency syndrome (test group) and 16 healthy persons (control group). The gas chromatography-mass spectrometry (GC-MS) was used to detect metabolic map, and orthogonal partial least squares discriminant analysis (OPLS-DA) was used to discriminate patterns and screen differentially metabolites. Results The urinary metabolite profiles of the two groups were significantly different. Compared with the control group, the leucine, proline and lysine levels were up-regulated in the experimental group, and the levels of succinic acid and hippuric acid were down-regulated. Conclusion The urine metabolic profiles of RNS patients with spleen and kidney yang deficiency syndrome changes. The five metabolites that have changed may be may be biomarkers of the disease. The differential metabolites suggest that amino acid metabolism, disturbance of tricarboxylic acid circulation and abnormal liver and kidney functions occur in the body.
