超声造影对兔单侧输尿管梗阻及再通后肾的动态功能研究

         

摘要

目的 利用超声造影(CEUS)及定量分析(QLAB)技术对兔单侧输尿管梗阻及不同时间点再通后梗阻侧肾做动态功能研究.方法 实验兔40只随机分为4组,利用血管封堵器制作右侧输尿管梗阻可再通模型,分别于UUO术前、术后3(A组)、7(B组)、14(C组)、28(D组)d及RUUO术后1个月对梗阻侧肾皮质行CEUS及QLAB检查.结果 UUO术后,随着梗阻时间的延长,梗阻侧肾皮质较术前强化程度逐渐减低、灌注速度逐渐减慢、造影剂廓清时间逐渐延长;RUUO术后1个月除D组外,A、B、C3组与同组梗阻解除前比较均有不同程度恢复,A组恢复最显著,B组次之,C组最不明显(P<0.05).结论 梗阻时间是影响梗阻侧肾脏功能恢复的关键因素;CEUS及QLAB能对肾脏血流灌注水平做动态研究,从而反映梗阻侧肾脏不同时间点功能变化.%Objective CEUS and QLAB were used to observe the rabbit renal function of the unilateral ureteral obstruction and recanalization at the different time.Methods Divided randomly the adult healthy 40 rabbits into 4 groups,the vasculature was made the model of the right ureteral obstruction,recanalization was performed at 3,7,14,28 days after the obstruction,a quantitative analysis of CEUS and QLAB was performed in the preoperative,postoperative and recanalization 1 month later RUUO.Results With the extension of obstruction time,the degree of enhancement,perfusion and contrast agent clearance rate were decreased and extended.The renal function of the ureteral obstruction 3 days,7 days,14 days was restored to normal level in different degree after the RUUO 1 month except D.Conclusions The duration of ureteral obstruction is the key factor affecting the recovery of renal function;CEUS and QLAB were able to perform real-time and dynamic research on the blood perfusion level of the kidneys to reflect the changes of renal function at different time points.
