上气道测压阻塞定位辅助治疗OSAHS的疗效观察

摘要

目的 探讨上气道测压联合手术治疗阻塞性睡眠呼吸暂停低通气综合征患者的疗效.方法 选取2013年12月-2015年12月我院治疗的中重度OSAHS患者75例,我院采用上气道测压结合常规耳鼻喉镜等检查,综合判定患者睡眠中的阻塞部位,分为腭后区阻塞48例、舌后区阻塞27例.腭后区阻塞患者进行保留悬雍垂UPPP手术;舌后区阻塞患者进行保留悬雍垂UPPP联合舌根低温射频消融术TCRFA.随访6个月,计算手术成功率;患者术前与术后6个月的BMI、AHI、LSaO2,ESS、VAS、QSQ比较.结果 腭后区阻塞手术成功率97.92,舌后区阻塞手术成功率92.59%;所有患者术后6个月BMI、AHI、LSaO2比术前均明显改善,差异有统计学意义(P<0.01);所有患者术后6个月ESS、VAS、QSQ比术前均明显改善,差异有统计学意义(P<0.01).结论 上气道测压结合常规检查,临床症状改善,术后成功率较高,并发症少,血氧饱和度提高,睡眠质量提高.%Objective To explore the effect of upper airway pressure measurements combined with operation in the treatment of patients with obstructive sleep apnea-hypopnea syndrome. Methods To select 75 cases with moderate-severe OSAHS treated in our hospital from December 2013 to December 2015. They were divided into 48 cases blocking in the posterior palatine area and 27 cases blocking in the posterior lingual area by comprehensive judge of upper airway pressure measurements combined with conventional ENT lens check. The posterior palatine area blocking patients achieved UPPP operation with uvula preserved, while the posterior lingual area blocking patients chose UPPP operation with uvula preserved combined with tongue base radiofrequency ablation at low temperature TCRFA. To compute operation success rate after following up for 6 months. The patients were compared with preoperative and postoperative BMI, AHI, LSaO2, ESS, VAS and QSQ. Results The success rate of the posterior palatine area blocking operation was 97.92%, which was 92.59% in the posterior lingual area blocking group. BMI, AHI, LSaO2, ESS, VAS and QSQ in all postoperative patients were significantly improved, the difference was statistically significant (P<0.01). Conclusion Upper airway pressure measurements combined with the routine inspection can improve clinical symptoms. It has high postoperative success rate and fewer complications.It can improve blood oxygen saturation and sleep quality.
