CT 引导下神经根脉冲射频联合局部注射复方倍他米松治疗腰椎间盘突出症

Objective-To-observe-the-therapeutic-effects-of-CT-guided-transforaminal-pulsed-radiofrequency-and-epidural-injection-of-betamethasone-to-the-nerve-root-for-the-treatment-of-lumbar-intervertebral-disc-protrusion-.-Methods-From-May-2012-to-February-2013,-46-patients-with-lumbar-intervertebral-disc-protrusion-were-treated-.Under-the-CT-guidance-,-the-affected-nerve-roots-were-accurately-positioned-.-Minimally-invasive-transforaminal-pulsed-radiofrequency-combined-with-epidural-injection-of-1-ml-betamethasone-and-1-ml-normal-saline-were-carried-out-.-Results-The-procedure-was-completed-smoothly-in-all-the-patients-,-without-complications-.Postoperative-follow-up-for-6-months-showed-,-according-to-the-modified-MacNab-standards-,-excellent-results-in-28-cases,-good-in-12-cases,-fair-in-4-cases,-and-poor-in-2-cases,-respectively,-with-an-excellent-or-good-rate-of-87.0%(40/46)-and-an-effective-rate-of-95.7%-(-44/46-)-.-Conclusion-CT-guided-transforaminal-pulsed-radiofrequency-and-epidural-injection-with-betamethasone-to-the-nerve-root-appears-to-be-an-effective-and-relatively-safe-interventional-technique-for-lumbar-intervertebral-disc-protrusion.%目的：探讨CT 引导下神经根脉冲射频加神经根周围注射倍他米松治疗腰椎间盘突出症的疗效。方法2012年5月～2013年2月，对46例腰椎间盘突出症，在CT导引下精确定位神经根后，突出椎间盘节段神经根脉冲射频治疗，相应椎间孔内神经根周围注射倍他米松1 ml＋生理盐水1 ml。结果均顺利完成治疗，无并发症。术后6个月随访，按改良MacNab标准：优28例，良12例，可4例，差2例，优良率87．0％（40/46），有效率95．7％（44/46）。结论神经根脉冲射频加神经根周围药物注射是腰椎间盘突出症的一种微创、有效、安全的治疗方法。
