Objective To investigate the indications,advantages and limitations of occipital craniotomy through transtentorial approach for thalamus tumors.Methods The clinical data of 16 patients undergoing resection of thalamic tumors via the occipital transtentorial approach were analyzed retrospectively.The tumor was resected through the posteroinferior space of the vein of Galen in 12 cases,combined para corpus callosotomy in 3 and corpus callosotomy through midline in 1.Results Total resection of tumor was achieved in 12 cases,subtotal resection in 1 and partial resection in 3.There was 1 perioperative death and 1 mild hemiplegia.Conclusions The occipital craniotomy through transtentorial approach is suitable for resection of the posteroinferior tumor of the thalamus,with the advantages of accurate positioning and small damage.%目的 探讨枕部开颅经小脑幕入路切除丘脑肿瘤的适应证及优缺点.方法 回顾性分析16例采用枕部开颅经小脑幕入路手术治疗丘脑肿瘤病例资料.经大脑大静脉后下间隙切除肿瘤12例,联合胼胝体旁切开3例,主要经胼胝体旁切开1例.结果 肿瘤全切除12例,次全切除1例,大部分切除3例.围手术期死亡1例,轻偏瘫1例.结论 枕部开颅经小脑幕入路适宜切除丘脑后内下部肿瘤,具有定位准确、创伤小等优点.
