纵向医联体建设背景下基层医务人员职业倦怠现状及影响因素分析

         

摘要

目的:在纵向医联体建设背景下, 调查不同类型的医联体内基层医务人员的职业倦怠现状, 并分析其影响因素.方法:选取北京市F区内8所社区卫生服务中心的基层医务人员作为调查对象进行问卷调查, 职业倦怠测定采用马氏工作倦怠量表-服务行业版, 问卷结果通过SPSS 19.0进行统计学描述和分析, 利用K-W检验分析不同医联体类型下基层医务人员职业倦怠情况的差异;利用Logistic回归分析医务人员的社会人口学特征、医联体类型、工作强度等因素对职业倦怠情况的影响.结果: (1) 基层医务人员处于中等程度的情感耗竭, 低等程度的人格解体, 中等水平的成就感降低. (2) 医联体的连接程度越紧密、夜班次数越多、工作时间越长, 基层医务人员的职业倦怠程度越高.结论:紧密型医联体中的基层医务人员的职业倦怠感较高, 可以通过绩效薪酬制度改革激励基层医务人员的工作积极性.%Objectives: To investigate the current situation of occupational burnout about primary health care staff among different types of medical association and analyze the influencing factors under the background of vertical medical association construction. Methods: The primary medical staffs in 8 community healthcare centers in Beijing F district were selected as the survey respondents. The occupational burnout was measured by the MBI-HSS scale, results had been analyzed by SPSS19.0, K-W test were carried out to analyze the gaps of job burnout and Logistic regression was used to analyze the influence of demographic features, category of medical association, labor intensity on job burnout. Results:Primary medical staffs are at a moderate level of emotional exhaustion, a low level of depersonalization, and a moderate level of lower achievement. The more closely the degree of connection of the medical integration, the greater the number of night shifts, and the longer the working hours, the higher the degree of burnout of the medical staff. Conclusion: The occupational burnout of the primary medical staff in the compact medical integration is high, which can motivate the work enthusiasm of primary medical staff through the reform of the performance pay system.
