Objective To investigate the velamentous placenta hy color Doppler ultrasound. Methods The sonographic features and clinical data of 35 pregnant women with velamentous placenta delivering in our hospital from January 2007 to August 2010. Results In 35 cases of velamentous placenta, there were 22 cord velamentous insertion, 6 vasa praevia, 6 bigeminal, 1 accessory placenta. Before 28 weeks of pregnancy, the sonographic diagnose accordance rate was 96. 6％ ( 22/23 ),the display rate of umbilical cord attachment point was 100. 0％ ( 23/23 ), 1 cases was misdiagnosed, while after 28 weeks of pregnancy, the accordance rate was 25. 0％ ( 3/12 ), the display rate was 50. 0％ ( 6/12 ), 3 misdiagnosed, 6 missed diagnoses. The difference was significant ( P ＜ 0. 05 ). Conclusion Color Doppler ultrasound is an important method in diagnosis of velamentous placenta. Before 28 weeks of pregnancy, screening can improve velamentous placenta detection, to provide important information for clinic , thus to reduce perinatal mortality rate.%目的 探讨帆状胎盘的彩色多普勒超声声像图特征.方法 回顾分析2007年1月-2010年8月在本院分娩的35例帆状胎盘孕妇的彩色多普勒超声声像图特点及临床资料.结果 35例帆状胎盘中,脐带帆状附着型22例,前置血管型6例,双胎型6例,副胎盘型1例.妊娠28周前超声诊断符合率为96.6%(22/23),脐带植入点显示率为100.0%(23/23),误诊1例;妊娠28周后超声诊断符合率为25.0%(3/12),脐带植入点显示率为50.0%(6/12),误诊3例,漏诊6例.妊娠28周前及妊娠28周后超声诊断符合率及脐带植入点率比较,差异有统计学意义(P＜0.05).结论 彩色多普勒超声是诊断帆状胎盘的重要检查方法,妊娠28周前筛查能提高帆状胎盘检出率,给临床提供重要信息,降低围生儿死亡率.
