社区家庭责任医生签约模式在慢性非传染性疾病中的应用效果

摘要

目的：探讨家庭责任医生签约模式在慢性非传染性疾病(慢病)中的应用效果。方法：选择参加社区家庭责任医生签约服务的慢性非传染性疾病患者400例，观察患者的家庭行为方式改变率、慢性非传染性疾病知识知晓率及用药依从性，比较干预前后的健康状况评分和自我管理行为。结果：干预6个月后，患者及家庭成员的家庭行为改变率91.25%，慢性非传染性疾病知识知晓率96.75%，服药依从性75.50%，自我管理行为和健康状况评分显著高于干预前(P＜0.05)。结论：社区家庭责任医生签约模式在慢性非传染性疾病管理中的效果显著，能够改善患者的健康状况，加强社区慢病的管理与控制。%Objective:To explore the application effect of community family responsibility doctor contract model in chronic non-communicable diseases.Methods:400 patients with chronic non-communicable diseases received community family responsibility doctor contract model were selected.We observed the patient's family behavior pattern change rate,chronic disease knowledge awareness rate and medication compliance,and compared the health status score and self management behavior before and after intervention.Results:6 months after the intervention,the rate of family behavior change of patients and family members was 91.25%;the chronic disease knowledge awareness rate was 96.75%;the medication compliance was 75.50%;self management behavior and health status scores were significantly higher than those before intervention(P<0.05).Conclusion:The application effect of community family responsibility doctor contract model in chronic non-communicable diseases was significant.It can improve the health of patients and improve the management and control of chronic diseases in community.
