数字化建设推进哮喘病网络管理模式的创新

         

摘要

目的:依据支气管哮喘(简称哮喘)专业特点,从当前数字化医院建设中存在的哮喘管理问题及需求出发,创新哮喘管理模式。方法:课题依托医院数字化的信息平台及六级病历系统,通过建立“哮喘的数字化网络管理平台”,进行医疗与管理的充分融合。结果:采用数字化网络管理模式对哮喘患者实施集中管理,定期短信提醒复诊督导及通知集中宣教,利用微信及QQ群与患者相互沟通,交流灵活,使患者可随时随地的接受健康指导、病情评估、随访提醒等,突破了空间的限制,同时提高了患者用药依从性,促进医患互动,大大提高了医护工作质量和效率,全面实现医院对慢性病管理的规范化、科学化、精细化,提高医院的管理水平。结论:改进现有的哮喘管理模式,探索一种医、护、患三方紧密联系的医疗服务模式,切实推广并落实哮喘的规范化治疗和管理,对提高哮喘的总体控制水平具有重要的意义,值得进行开展及推广。%Objective: On the basis of professional characteristics of bronchial asthma (hereafter referred to as asthma), starting from asthma management problems and demand existing in the current digital hospital construction, to innovate the asthma management mode. Methods: Relying on the digital information platform and six levels of medical record system of the hospital, through the establishment of the digital network management platform of asthma, the subject fully integrates healthcare and management. Results:With digital network management mode, centralized management of patients with asthma is implemented, subsequent visit is reminded of and propaganda and education are informed of regularly with SMS, with WeChat and QQ group patients are communicated with, the communication is flexible, so that the patients can accept health guidance, condition evaluation, follow-up reminder and others anytime and anywhere. It breaks the limit of the space, increases patients' medication compliance, promotes the doctor-patient interaction, greatly improves the medical work quality and efficiency, comprehensively realizes the hospital' s standardized, scientific and refined management of chronic diseases, and improves the management level of hospital. Conclusion:Improving existing asthma management mode, exploring a kind of medical service mode with doctors, nurses and patients closely related, and promoting and implementing the standardized treatment and management of asthma has significance for the improvement of the overall control level of asthma. They are worth carrying out and promoting.
