中上段食管癌超胸顶吻合的应用

摘要

Aim To evaluate the significance of the anastomosis above chest top in upper or middle esophageal carcinoma patients.rnMethods Eighty-two cases were divided into 2 groups randomly after August,2006. In group A(48 cases) ,anastomosis was conducted rnabove chest top,while neck anastomosis conducted in group B(34 cases). Results 0 case of recurrent laryngeal nerve were injury was rnfound in group A,while 6 cases in group B. Operation time in group A was (3.0 ± 0.3) ,while that in group B was ( 13.8 ± 0.8) , t=rn6.15. There was statistically difference (P ＜ 0.05) between the two groups. No positive case of upper cut margin was found in both rngroups. Conclusion It is recommended to take the anastomosis above chest top instead of the anastomosis at neck in upper or middle ernsophageal carcinoma patients,which not only can reduce operation time and the incidence of recurrent laryngeal nerve injury but also has rnthe advantages of simple operation and worthines of gromotion.%目的 探讨超胸顶吻合法在中上段食管癌中应用的意义.方法 随机选择该科自2006年8月以来,手术切除中上段食管癌82例.共分为A、B组.A组48例,采用超胸顶吻合;B组34例,采用颈部吻合.结果 喉返神经损伤 A组0例,B组6例;两组手术时间分别为(3.0±0.3),(3.8±0.8) h,t=6.15,P＜0.05,两组比较有统计学差异.两组上切缘无一例阳性.结论 在中上段食管癌,采用超胸顶吻合,明显减少手术时间和喉返神经损伤的发生,而且操作简单,易于推广.
