冠心病合并2型糖尿病的临床及冠状动脉病变特点分析

摘要

目的 探讨冠心病合并2型糖尿病的危险因素和冠状动脉病变的特点.方法 选取120例经冠脉造影确诊的冠心病患者,其中60例为冠心病合并糖尿病患者,60例为同期住院的非糖尿病冠心病患者,对两组的临床表现,合并的危险因素,血脂水平及冠状动脉造影分别进行比较.结果 冠心病合并2型糖尿病患者高血压发生率明显高于不伴糖尿病患者(P＜0.05);吸烟,无痛性心肌缺血及心梗的发生率两组间差异无显著性(P＞0.05).冠心病合并2型糖尿病患者总胆固醇(TC),甘油三脂(TG)和低密度脂蛋白胆固醇(LDL-C)的水平明显高于无糖尿病的冠心病患者(P＜0.05),而高密度脂蛋白胆固醇(HDL-C)的水平两组间差异无显著性(P＞0.05);冠心病合并2型糖尿病患者与非糖尿病冠心病患者相比,冠状动脉病变主要表现为多支血管病变及弥漫性病变(P＜0.05).结论 冠心病合并2型糖尿病患者的高血压发生率、TC、TG和LDL-C均明显高于无糖尿病的冠心病患者,前者冠状动脉的病变也比后者严重.%Objective To investigate the risk factors and the pathological features of coronary artery diseases with type II diabetes. Methods A total of 120 patients with coronary artery disease which were finally diagnosed by coronary artery angiography were recruited. These patients were randomly divided into diabetes group( re = 60 )and non-diabetes group( n = 60 ). Clinical and angiography characteristics, risk factors and blood fat levels were compared between the two groups. Results In diabetes group,the incidence rate of hypertension was significantly higher than that in non-diabetes group ( P <0. 05 ). There were no remarkable difference of the incidence rate of smoking, silent myocardial ischemia and myocardial infarction between the two groups( P >0. 05 ). The levels of total cholesterol, triglyc-eride and low density lipoprotein-cholesterol( LDL-C )in the diabetes group were significantly higher than that in the non-diabetes group ( P<0. 05 ). However, no significant differences in levels of high density lipoprotein cholesterol( HDL-C )were found between the two groups ( P>0.05 ). In addition, when in contrast to non-diabetes group, multi-vessel lesion and diffused lesion of coronary artery were more common in diabetes group ( P < 0. 05 ). Conclusion The incidence rate of hypertension and the levels of TC, TG, LDL-C were significantly higher,synchronously with severer coronary lesions in coronary artery disease with type II diabete patients.
