Objective To evaluate the effect evaluation of comprehensive nursing for the patients with uremia combined with heart failure treated by hemodialysis. Methods 130 cases with uremia treated by hemodialysis were randomly divided into control group and experience group. 60 patients of control group were given routine nursing for hemodialysis,70 cases of experience group received psychological nursing and preventing cardiovascular complications based routine care. The occurrence of heart failure, SDS scores,6-minute walking distance , LVEF and FS were compared between two groups. Results The occurrence of heart failure were 31.7% ( 19/60 )and 52. 9% ( 37/ 70 )in experience group and control group respectively with significant difference( P <0. 05 ) ;SDS scores after intervention in experience group were much lower than those in control group[ ( 34. 68 ±5. 63 )vs( 40. 26 ±6. 12 ),P <0. 05 ];Compared with control groups-minute walking distance in experience group greatly increased[ ( 418. 32 ±70. 23 )m ra( 369. 93 ±62. 18 )m,P <0. 05 ];Blood pressure, heart rate,LVEF and FS after intervention in experience group were superior to those in control group( P <0. 05 ). Conclusion Comprehensive nursing for the patients with uremia treated by hemodialysis can greatly decrease the occurrence of heart failure, relieve the negative emotion and improve cardiac function.%目的 评价综合护理对尿毒症患者血液透析期间合并心衰的效果.方法 将该院130例尿毒症进行血液透析患者随机分为对照组与观察组,给予60例对照组患者常规血液透析护理,70例观察组患者在常规护理基础上给予心理护理及预防心血管并发症的护理措施.比较两组心衰的发生率、心衰患者治疗前后SDS评分、6 min步行距离及左室射血分数(LVEF)、左心室短径缩短(FS)的差别.结果 观察组与对照组心衰发生率分别为31.7%(19/60)和52.9%(37/70),差别具有统计学意义(P＜0.05);观察组心衰患者干预后SDS评分显著少于对照组[(34.68±5.63)vs(40.26±6.12),P＜0.05],6 min步行距离显著长于对照组[(418.32±70.23)m vs(369.93±62.18)m,P＜0.05];干预后观察组血压及心率显著优于对照组(P＜0.05);观察组心衰患者LVEF及FS指标显著优于对照组(P＜0.05).结论 对于尿毒症血液透析患者综合护理,可显著减少心衰发生率,减轻心衰患者负性情绪,改善心功能.
