An accident investigator's perspective

Although it is often said that human error is responsible for about 80 percent of all marine accidents the actual percentage is almost certainly higher. Understanding the human factor dimension in the causes of accidents is a fundamental part of the marine accident investigator's task and this paper gives a general view of how this features in the day to day work of inspectors in the UK's Marine Accident Investigation Branch.
