The number of available surgical treatments has increased over the last decadeThere is still uncertainity about the best treatment because of lack of comparative,randomized studies using rigid inclusion criteria and objective outcome measuresSurgical treatment for DLSS can be broadly classified into decompression of lumbosacral nerve roots with and without stabilization. Decompression without stabilization can be performed with dorsal laminectomy and lateral foraminotomy of the L7/S1 neurovascular foramen. These procedures may be used alone or in combination. Decompression with stabilization of the lumbosacral junction include placement of screws through the articular facets of L7/S1. Recently, more rigid fixation procedures have been described including pedicle screw-rod fixation (PSRF) and in a cadaveric study the biomechanical results of lumbosacral stabilization using an interbody cage and PSRF were presented.
