Unfortunately, loss events continue. Common threads have renewed focus on the following deficiencies impacting Safe Automation: 1. Invalid assumptions in specification 2. Lack of maintenance or timely repair 3. Improper use of bypasses 4. Changes not recognized, not documented, or not tested adequately prior to startup 5. Inadequate training of operators or of staff 6. Failure to resolve PHA/Functional Safety Audit findings.
