Case report: IV Dexmedetomidine infusion as the sedative agent for a 16 hour plastic surgery under combined spinal-epidural anaesthesia.

We report the case of a 42 year old man, ASA 2 who underwent a 16 hours long operation for a total ankle free flap and SSG under exclusively combined spinal-epidural (CSE) anaesthesia. We used IV Dexmedetomidine infusion (0.3-0.7ng/kg/hr) supplemented by low dose boluses of midazolam and fentanyl as our sedative agent throughout the procedure. Epidural infusion was continued post operatively for pain control as well as to maintain the tissue flap's perfusion. This technique provided safe anaesthesia, optimal surgical condition and excellent patient satisfaction.
