Functional recovery after mandibulectomy for cancer has significantly improved with the use the free fibula osteocutaneous flap. However, some drawbacks include the increased operative time required to accurately bend the reconstruction plate to the native mandible as well as the time associated with performing and revising the closing wedge osteotomies to obtain good contact between the bone edges. We developed a technique to reduce the intraoperative time associated with these maneuvers as well as to improve the accuracy of the initial osteotomies and minimize revision of the osteotomies. The authors describe their technique for mandibular reconstruction using preoperative virtual planning, stereolithographic models, prebent plates, and osteotomy guides through illustration with a case report. The described technique of mandibular reconstruction allows for improved efficiency and precision with restoration of excellent native mandibular contour.
