Posterior Perineal Block for Minor Anorectal Surgery

The aim of the study was to analyze the safety and efficacy of posterior perineal block minor anorectal surgery. Materials and methods. From January 2003 to April 2005 383 operations were performed under posterior perineal block: closed haemorrhoidectomy was performed in 286 patients, lateral sphinc-terotomy in 73, fistulotomy in 12, polypectomy in 12, removal of condylo-mata in 1. Perineal block was performed using the mixture of bupivacaine and lidocaine solutions. Results. No premedication, sedation or involvement of anaesthetic team was necessary. 74% of patients were discharged on the same day, and the remaining 25 were hospitalized for 2-14 days (usually for other conditions). Six complications occurred: 3 cases of bleeding, 2 perianal abscesses, and prolonged perianal pain in one case. In 4 cases reoperation was necessary: in two repeated suturing and in 2 - drainage of an abscess. No cases of urinary retention occurred in our series. Conclusions: Posterior perineal block was safe and effective anaesthesia for minor anorectal operations, well suited for day-case or ambulatory surgery.
