DESIGN AND CHARACTERIZATION OF A NEW SYNTHETIC ACRYLIC ADHESIVE FOR SQUINT MUSCLE EYE SURGERY

The main advantage of the use of adhesives to join human tissues as compared to the traditional suture, is the creation of homogeneous and uniform distribution of stresses all along the joint. Furthermore, the technology of adhesives is easy to apply, reduces the surgical time, avoids the damage produced by suture, inhibits scares formation, and there is no need to remove the remaining sutures or bandages after surgical practice. Since 1950, cyanoacrylate adhesives have been used in surgical practice. However, adhesives have been scarcely used in ophthalmic surgical practice. The application in eye surgery was not properly developed mainly due to the stiffness of the current adhesives. Several cyanoacrylate adhesives exhibit excellent performance (Dermabond, Tissuacryl, Notably), although none of them are flexible enough to allow the joining of the sclera to the squint muscle in eye surgery. Gasset et al. and Robin et al. tested different cyanoacrylate adhesives for corneal surgery, and they stated the existence of chronic inflammatory reaction. Therefore, the objective of this study is the synthesis and characterization of new bioadhesive formulation, based on the mixture of a new acrylic derivative (ethyl carboxyacrylate) and ethyl cyanoacrylate, for the joining of the squint muscle and the sclera in eye surgery. This mixture is able to polymerize in the presence of important amounts of water producing less exothermal reaction than cyanoacrylate adhesives, and the polymer obtained after curing is less stiffer (i.e. more flexible). The ethyl cyanoacrylate-ethyl carboxyacrylate mixtures were previously used in practical surgery and were compared to suture performance. Excellent performance was shown by using the adhesive mixtures.
