The dominant part of ship accidents is constituted by powered grounding, in cost if not in loss of lives. Groundings are mainly caused by human failure. Over the years, numerous efforts have been made to reduce or eliminate human failures. From the point of view of risk analyses, risk reduction could be attained by reduction of frequency of human failures, and/or by reduction of grounding consequences. The latter is not a subject of this paper.
