Endothelin-1 associated with intramural coronary artery disease in pigs with hypertrophic cardiomyopathy

Intramural coronary artery disease (ICAD) has been considered one of the important histopathologic features in spontaneously occurring hypertrophic cardiomyopalhy(HCM) in humans, pigs, cats and dogs. The characteristics of histopathologic lesions ofICAD are perivascular fibrosis, increased wall thickness, and narrowed lumen. The purpose of this study was to localize and define smooth muscle cells (SMC) of ICAD and determine endothelin-1 which stimulates proliferation of SMC in the arterial wall. Our findings were: Thethickening of the wall and narrowing of the lumen of intramural coronary arteries were caused by the proliferation of SMC and increase of collagen-rich extracellular matrix. SMC migrated through disruptured internal elastaic membranes into the intitna. The concentration of endothelin-1 was incerased in the left ventricular free wall and septum in pigs with HCM. This study suggested that ICAD in spontaneously occurring HCM in pigs was induced by remarkable proliferation of SMC and possible collagen fibres through fragmented internal elastic membrane into the intima. The distribution of endothelin-1 in heart is correlated with lesions of ICAD in pigs with HCM.
