CHANGES IN MAINTENANCE AS THE RESULT OF EFFECTIVE ROOT CAUSE ANALYSIS

This paper describes the root cause analysis (RCA) process and the tools use in implementation. RCA is not just for catastrophic equipment failures but can be used to avoid the catastrophic failures if used as a course on "Normal" equipment maintenance. RCA is a team sport. Not only is it faster; it tends to have a higher quality outcome if used correctly. Root cause investigation is probably the most critical step and one that can and will get better over time if used consistently with RCA. By bringing all the information together, the investigation will arrive at the root cause and, most importantly, implement the follow up ascertaining the root cause.
