    • 摘要: BACKGROUND Current literature shows no clear answer on the question how to manage the capsule after hip arthroscopy.Regarding patient reported outcome measures there seems to be no difference between capsular repair or unrepaired capsulotomy.AIM To evaluate and compare the integrity of the hip capsule measured on a magnetic resonance imaging(MRI)scan after capsular repair or unrepaired capsulotomy.METHODS A case series study was performed;a random sample of patients included in a trial comparing capsular repair vs unrepaired capsulotomy had a postoperative MRI scan.The presence of a capsular defect and gap size were independently evaluated on MRI.RESULTS A total of 28 patients(29 hips)were included.Patient demographics were comparable between treatment groups.There were 2 capsular defects in the capsular repair group and 7 capsular defects in the unrepaired capsulotomy group(P=0.13).In the group of patients with a defect,median gap sizes at the acetabular side were 5.9 mm(range:2.7-9.0)in the repaired and 8.0 mm(range:4.5-18.0)in the unrepaired group(P=0.462).At the muscular side gap sizes were 6.6 mm(range:4.1-9.0)in the repaired group and 11.5 mm(range:3.0-18.0)in the unrepaired group(P=0.857).The calculated Odds ratio(OR)for having a capsular defect with an increasing lateral center-edge(CE)angle was 1.12(P=0.06).The OR for having a capsular defect is lower in the group of patients that underwent a labral repair with an OR of 0.1(P=0.05).CONCLUSION There is no significant difference in capsular defects between capsular repair or unrepaired capsulotomy.Regarding clinical characteristics our case series shows that a larger CE angle increases the likelihood of a capsular defect and the presence of a labral repair decreases the likelihood of a capsular defect.

    • 摘要: BACKGROUND Mycobacterium species(Mycobacterium sp)is an emerging cause of hip and knee prosthetic joint infection(PJI),and different species of this organism may be responsible for the same.AIM To evaluate the profile of hip and knee Mycobacterium PJI cases as published in the past 30 years.METHODS A literature search was performed in PubMed using the MeSH terms“Prosthesis joint infection”AND“Mycobacterium”for studies with publication dates from January 1,1990,to May 30,2021.To avoid missing any study,another search was performed with the terms“Arthroplasty infection”AND“Mycobacterium”in the same period as the previous search.The Preferred Reporting Items for Systematic Reviews and Meta-Analyses chart was used to evaluate the included studies for further review.In total,51 studies were included for further evaluation of thecases,type of pathogen,and treatment of PJI caused by Mycobacterium sp.RESULTS Seventeen identified Mycobacterium sp were reportedly responsible for hip/knee PJI in 115 hip/knee PJI cases,whereas in two cases there was no mention of any specific Mycobacterium sp.Mycobacterium tuberculosis(M.tuberculosis)was detected in 50/115(43.3%)of the cases.Nontuberculous mycobacteria(NTM)included M.fortuitum(26/115,22.6%),M.abscessus(10/115,8.6%),M.chelonae(8/115,6.9%),and M.bovis(8/115,6.9%).Majority of the cases(82/114,71.9%)had an onset of infection>3 mo after the index surgery,while in 24.6%(28/114)the disease had an onset in≤3 mo.Incidental intraoperative PJI diagnosis was made in 4 cases(3.5%).Overall,prosthesis removal was needed in 77.8%(84/108)of the cases to treat the infection.Overall infection rate was controlled in 88/102(86.3%)patients with Mycobacterium PJI.Persistent infection occurred in 10/108(9.8%)patients,while 4/108(3.9%)patients died due to the infection.CONCLUSION At least 17 Mycobacterium sp can be responsible for hip/knee PJI.Although M.tuberculosis is the most common causal pathogen,NTM should be considered as an emerging cause of hip/knee PJI.

    • 摘要: BACKGROUND Charcot neuroarthropathy(CN)is a systemic disease characterized by progressive bone loss and destruction,which is usually closely related to diabetes,HIV,etc.However,CN caused by syringomyelia accounts for only 5%of CN cases;the shoulder and elbow are most often involved,and the hip joint is rarely affected.As a rare factor,cervical spondylotic myelopathy(CSM)can be associated with syringomyelia,which is scarcely reported in the literature.Here,we present the first case report to date of CN of the hip caused by syringomyelia secondary to CSM.CASE SUMMARY We describe a 76-year-old male patient who was diagnosed with CSM due to neck pain and weakness of limbs 16 years ago.Four years ago,he noticed recurrent swelling of the right hip with pain and was diagnosed with degenerative arthritis.Recently,however,his symptoms gradually worsened,and because of progressive pain,destabilization and weakness of the right hip,he was admitted to our hospital.Through systematic physical,radiographic and laboratory examinations,we finally reached a diagnosis:CN of the right hip associated with syringomyelia secondary to CSM.After comprehensive evaluation of the patient's condition,we performed right total hip arthroplasty.During the follow-up,the patient felt well clinically and could walk independently with a knee brace.CONCLUSION We suggest a possible etiological association between CSM and syringomyelia,which may reflect a potential pathogenesis of CN.We encourage clinicians to actively carry out a detailed medical history and comprehensive physical and imaging examinations in patients with joint lesions,especially chronic shoulder neck pain,to rule out the possibility of this association,which plays a crucial role in the early diagnosis of CN.Arthroplasty may no longer be an absolute contraindication to surgical treatment of CN.Reasonable selection of the surgical strategy can markedly improve the clinical symptoms and quality of life of patients.

    • 摘要: Background: Total hip arthroplasty (THA) is a common procedure that is increasingly being performed in younger patients. Deep acetabular reaming will result in more bone loss and the need for large acetabular components to be implanted. It can also lead to impingement, loosening, an altered center of rotation, and intraoperative periprosthetic fracture. The purpose of this study is to determine whether the single ream, robotic arm-assisted (RAA) THA can preserve a greater volume of bone stock compared to conventional hip replacement and resurfacing. Methods: We prospectively recruited 69 patients who had undergone primary THA using the Stryker Trident Acetabular System?in combination with the Stryker RAA System (MAKO)?and compared their mean reaming weight (g) with that of conventional hip replacement and resurfacing, as measured by Brennan et al. Comparison of acetabular reaming during hip resurfacing versus uncemented THA (J Orthop Surg. 2009;17(1): 42-46). Results: The mean reaming weight using the MAKO system was 9.08 g, which was 29% less than the reaming weight using uncemented THA and hip resurfacing of 12.75 g. None of the acetabular cups required screw fixation. During the 35-month follow-up period, there were no complications related to cup placement or positioning. Conclusions: The use of RAA THA results in statistically significant preservation of acetabular bone compared to conventional hip replacement and resurfacing. This approach reflects the increased precision offered by RAA single reaming. Surgeons may consider utilizing RAA THA, particularly in younger patients, to better preserve bone stock as this could potentially impact future revision procedures.

    • 摘要: BACKGROUND Neurogenic heterotopic ossification is an acquired serious complication described in patients with central nervous system disorders and defined by bone formation in non-osseous tissue.CASE SUMMARY We present an unusual case of a 13-yr-old boy presenting with hip pain and severe gait impairment 5 mo after the diagnosis of hemiplegia following a spontaneous intracerebral haemorrhage.Computed tomography revealed bilateral heterotopic ossification of both the paretic and the non-paretic limbs,with entrapment of the sciatic nerve.The choice of surgical or nonsurgical management of such patients depends on the timing of diagnosis,the symptoms,and the extent of maturation of the ossified lesions.Surgical resection remains the only treatment with proven,evidence-based effectiveness.The choice of surgical approach largely depends on the location of the ossified lesions.CONCLUSION We believe the plane of dissection presented is a satisfactory option for resection of a posteromedial mass and sciatic nerve release.

    • 摘要: 【目的】本文在国产加速卡异构平台上,对基于BH-树方法和粒子网格方法的并行天文N体模拟软件Gadget-2进行了移植优化。【方法】基于HIP将Gadget-2中最耗时的短程力计算部分移植到加速卡上,包括本地树的遍历,并对结构体数组进行重构,同时充分利用寄存器与共享内存,提高设备端的访存效率。【结果】数值结果表明,移植优化的版本整体性能加速13.27倍,短程力计算加速35.67倍,并行效率达到57.29%,功率谱结果验证了移植优化的正确性。【结论】本文实现了天文N体模拟软件Gadget-2在加速卡异构平台上的移植和优化,并为大规模宇宙学模拟提供支撑。

    • 摘要: 为分析HSC70互作蛋白(HSC70 interacting protein,简称HIP)基因在黏虫生殖中的潜在作用,采用RT-PCR的方法从黏虫中克隆1个HIP同源基因的全长cDNA序列,并采用实时荧光定量PCR分析其在黏虫不同发育阶段(卵、1~6龄、蛹和4日龄雌虫)和4日龄雌虫不同组织[头(无触角)、胸、触角、翅、中肠、脂肪体和卵巢]中的表达特性。结果表明,克隆得到的HIP序列全长为1215 bp,命名为MsHIP。MsHIP编码405个氨基酸(aa),预测的分子量为44.04 ku。序列分析表明,MsHIP具有定义HIP的3个保守结构域,即N-端二聚体结构域,3个TPR结构域和C-端U-box结构域。系统发育分析显示黏虫HIP与棉铃虫H.armigera HIP亲缘关系最近。时空表达分析表明,MsHIP在所有发育阶段均表达,并在4日龄雌成虫中的表达量水平最高,其表达量是对照的45.28倍;MsHIP 4日龄雌虫各组织中均表达,其中在卵巢中的表达量最高,脂肪体次之,其表达量分别为对照的18.55倍和13.58倍,表明MsHIP在黏虫的生殖中发挥重要作用。

    • 摘要: Objective:To investigate the clinical efficacy and perioperative safety of simultaneous or staged bilateral total hip arthroplasty(THA)using meta-analysis.Methods:Relevant retrieval methods were developed to retrieve PubMed,Embase,Cochrane Library,CNKI,Wanfang and VIP databases,and the time was set to build the database until September 2020.All the literatures related to simultaneous or staged total hip arthroplasty were screened out.The final included literatures were determined according to the inclusion and exclusion criteria,and the quality of the literatures was evaluated.Meta analysis of all indexes was performed using Review Manager 5.3 software.Results:A total of 22 articles were included,including 3415 patients.Meta analysis results showed that the total hospitalization time in the simultaneous group was significantly lower than that in the staged group[MD=-9.99,95%CI(-14.72,-5.26),P<0.0001].Total hospitalization expenses in the simultaneous group were lower than those in the staged group[MD=-1.3,95%CI(-1.73,-0.86),P<0.00001].The operative time in the simultaneous group was less than that in the staged group[MD=-27.08,95%CI(-40.89,-13.26),P=0.0001].The total blood loss in the simultaneous group was less than that in the staged group[MD=-176.55,95%CI(-282.5,-70.6),P=0.001].The surgical transfusion volume in the simultaneous group was higher than that in the staged group[MD=69.85,95%CI(40.48,99.22),P<0.00001].Postoperative Harris score of hip joint in the simultaneous group was higher than that in the staged group[MD=1.79,95%CI(0.8,2.79),P=0.0004].The incidence of operative complications in the simultaneous group was lower than that in the staged group[OR=0.73,95%CI(0.56,0.96),P<0.05].Conclusion:Compared with staged bilateral total hip replacement,simultaneous bilateral total hip replacement has advantages in terms of reducing hospitalization time,saving hospitalization expense,shortening operation time and reducing surgical bleeding amount,and can promote postoperative hip function recovery,reduce postoperative complications,but increase total blood transfusion volume.
