摘要:
目的 探讨微量注射泵远程自动推注造影剂在子宫输卵管造影检查中的优势及应用价值.方法 选择2015年6月-12月在河北省某市妇幼保健院放射科行子宫输卵管造影的80例不孕症患者,采用随机数字表法分为观察组和对照组各40例,观察组采用微量注射泵远程自动推注造影剂进行子宫输卵管造影检查,对照组采用临床医师床旁推注造影剂进行子宫输卵管造影检查.对比分析两组患者的造影质量,子宫输卵管阻塞诊断的灵敏度、特异度及符合率等,以及造影中不良反应的发生情况.结果 观察组总体影像质量及HSG成功率均明显优于对照组,差异有统计学意义(P<0.01).观察组检查结果与腹腔镜下通染液检查结果Kappa值为0.750,一致性较好,对照组影像检查与腹腔镜下通染液检查结果Kappa值为0.471,一致性一般.对照组推注造影剂速度较快,平均为(48.6± 10.3)s,观察组推注造影剂速度均匀,平均为(147.2±15.4)s.观察组推注过程中恶心呕吐、腹痛发生率均低于对照组,其中腹痛发生率明显低于对照组,差异有统计学意义(P<0.05),且均在造影结束后自动缓解.结论 应用微量注射泵行子宫输卵管造影,可以快速、高效及动态观察造影过程,能够提高子宫输卵管图像质量,可提高子宫输卵管疾病的诊断准确率.%Objective To investigate the application value of hysterosalpingography examining with micro-injection pump pushing contrast medium remotely and automatically.Methods To select 80 cases infertility patients who were examined by hysterosalpingography in Radiology department in 2015.6 to 2015.12 as the research object.Using the random number table method,they were divided into observation group (n=40) and control group (n=40),the observation group patients were examined by hysterosalpingography with micro-injection pump pushing contrast medium remotely and automatically,the control group patients were examined by hysterosalpingography with bedside pushing contrast medium by clinician.Contrastively analyzed the imaging quality,the sensitivity,specificity,coincidence rate etc of the uterine and fallopian tube obstruction diagnosis,and the imaging adverse reactions of the two groups patients.Results The total image quality and the HSG success rate of the observation group were better than the control group,the difference had statistical significance (P<0.01).The inspection results of observation group and laparoscopic examination results had a good consistency,the inspection results of control group and laparoscopic examination results had a general consistency.The contrast agent injection rate of the control group was more fast,the average was (48.6 ± 10.3) s.The contrast agent injection rate of the observation group was well-proportioned,the average was (147.2 ± 15.4) s.The nausea and vomiting,abdominal pain incidence of observation group were lower than the control group in the process of injection,and the abdominal pain incidence of observation group was significantly lower than the control group,the difference had statistical significance(P< 0.05),and they were alleviated automatically at the end of the procedure.Conclusion Hysterosalpingography examining with micro-injection pump can quickly,efficiently and dynamically observe the imaging process,and improve the quality of the uterus oviduct image and the diagnosis accuracy of uterine tubal disease.
