The method involves filling questionnaires (10) relating to animal health status by veterinarians (12, 18) to study the object of a consultation in their cabinets, with an agreement from owners (28) of pet animals. A number of these questionnaires are studied to provide information (26) related to levels indicating the health status, treatment costs, and organization of a welfare program, to third parties via an Internet network. The third parties are the owners of the pet animals, veterinarians, insurance companies and/or pharmaceutical laboratories. Independent claims are also included for the following: (A) a system for preventing and monitoring diseases of pet animals (B) a server for preventing and monitoring diseases of pet animals (C) a form for preventing and monitoring diseases of a pet animal, destined to be filled by veterinarians treating the animals with an agreement from their customers.
