TECHNIQUE FOR SURGICAL ACCESS IN PATIENTS WITH GIANT CICATRICIAL VENTRAL HERNIA WITH MESO- AND HYPOGASTRIC LOCATION OF HERNIAL GATES AND PENDULOUS ABDOMEN

A technique for the surgical access in the patients with the giant cicatricial ventral hernia with meso- and hypogastric location of the hernial gates and the pendulous abdomen comprises performing two cuts lining transversally the hernial outpouching with the dissection of the angle-shaped fragment in the lower edge of the wound in the center of the abdomen with the postoperative cicatrice and the apex of the angle oriented downwards and the excess of the skin and subcutaneous fat base. In the top of the remaining tissue in the upper edge of the wound, two angle-shaped cuts are performed with the apex directed upwards with the symmetrical dissection of the tissue relative to the middle abdominal line. The total width of the fragments in the upper edge of the wound corresponds to the width of the fragment dissected in the lower edge. Finally, the wounds are sutured.
