PPROBLEM TO BE SOLVED: To solve the problem wherein existing independent issue of magnetic cards or IC cards, containing registration information from different medical institutions, makes it necessary to hold a plurality of cards for visiting a plurality of medical institutions at the risk of losing them and at the inconvenience of carrying. PSOLUTION: A portable terminal 1 comprises a nonvolatile storage part 105 for storing a plurality of consultation information files 14, each containing registration information including medical institution information, a registration number and a patient's personal information; a consultation information view display means 11 for prompting a medical institution to be selected and displaying the corresponding consultation information file 14; a data transfer means 13 for establishing transfer to a consultation management terminal 2 by serial I/F or short-range communication; and a consultation information reference and updating means 12 for receiving medical institution designation information from the consultation management terminal 2, reading out the consultation information file 14 corresponding to the designated medical institution from the nonvolatile storage part 105, sending it to the consultation management terminal 2, and updating the consultation information file with information from the consultation management terminal 2.
