A seed phantom for position accuracy evaluation of radiation seed is provided to secure the form of a discrete seed artificially in order to recognize the performance and limits of an algorithm distinctly. An outer holder(42) is a cylindrical. An accommodation hole(42a) is formed in the center of the outer holder. A load holder(44) is mounted to the accommodation hole of the outer holder. A fixing rod(46) is inserted into a mounting hole of the load holder. A reference line(42b) is formed on the outer circumference of the outer holder at every 90 degrees interval. The load holder is made of the same material as the outer holder. Nine mounting holes(44a) are formed on the load holder.
