PROBLEM TO BE SOLVED: To solve the problem, wherein elderly persons or physically disabled persons who are orally-challenged or hearing impaired and have difficulty in uttering words cannot convey their intentions when they receive care or in daily lives.;SOLUTION: In an intention-expressing sheet for indicating intention and a care book for expressing intention: each item is colored in different colors so as to be recognizable at a glance; words necessary for conducting living are collected; and words and colors are combined, by making use of color synesthesia so that a person to be cared can rapidly and easily convey the word the person wants to convey to a caregiver by pointing to the word with a finger.
