The present invention relates to a system and method for diagnosing a stroke lesion on the basis of an image and, more particularly, to a system and method for diagnosing a stroke lesion on the basis of a medical image for diagnosis of a stroke and prescription for the stroke. To this end, a system for diagnosing a stroke lesion on the basis of an image according to an embodiment of the present invention may comprise: a reception unit for receiving a diffusion-weighted image (DWI) and a perfusion-weighted image (PWI) of an examinee's body part containing a lesion from an imaging modality or a database; a registration unit for registering the diffusion-weighted image and the perfusion-weighted image; a user menu providing unit for providing a user menu which allows a user to determine display options for the diffusion-weighted image and the perfusion-weighted image; an input translation unit for translating the display options in response to a user input through the user menu; and a display control unit for displaying, on a screen, at least one of the diffusion-weighted image and the perfusion-weighted image according to the display options.
