A microcirculation resistance index (IMR) calculation method based on an angiogram image and a hydrodynamics model, comprising: selecting coronary angiogram images of two positions for carrying out segmentation so as to obtain the center line and diameter of a coronary artery (S01); generating a three-dimensional coronary artery model (S02); acquiring blood transit mean time (Tmn) and speed (S03); establishing a three-dimensional blood vessel grid; generating a two-dimensional axially symmetrical plane model on the basis of the center line and the diameter of the coronary artery from X-ray reconstruction, then establishing two-dimensional axially symmetrical blood vessel grids (S04); measuring mean blood pressure Pa of the aorta (S05); solving a fundamental formula of incompressible flows on the basis of the obtained blood flow speed and the generated three-dimensional blood vessel grid, calculating the pressure drop ΔPi of each point from the entrance to the downstream along the blood vessel center line, and obtaining the coronary artery far-end artery pressure Pd=Pa-ΔPi (S06); and calculating a microcirculation resistance index IMR=Pd*Tmn (S07). The described method does not require the use of pressure guide wires.
