To monitor subjects after discharge from a hospital.SOLUTION: Systems and methods of detecting exacerbation on the basis of dynamically updated thresholds use a dynamic threshold to determine whether a caregiver needs to be notified of exacerbation of conditions of disease of subjects. The subjects are chronic obstructive pulmonary disease (COPD) patients. The dynamic threshold is determined on the basis of values of a sequence consisting of values for a metric that quantifies characteristics of lungs of the subjects.SELECTED DRAWING: Figure 1
