FIELD: medicine.;SUBSTANCE: invention refers to medicine, particularly to paediatric cardiosurgery. It is performed by commissurotomy with entering the middle layer of fibrous ring of pulmonary arterial valve (AV). Tangential resection of pulmonary arterial valve edge flaps is performed. Fibrous subvalvular adhesions of arterial valves of the right ventricle (RV) are dissected. Guiding cuts are made in the projection of the anterior wall of the right ventricular outflow tract (RVOT) on the side of the right ventricle cavity, receding 3 mm from the projection of the pulmonary arterial valve. It is followed by balloon dilatation of fibrous ring of pulmonary arterial valve and delamination of leaflets. In particular case at breakage and detachment of valves of aircraft valve, reconstruction is performed by means of flap to base fixing, closure of flap rupture and, if necessary, compensation of insufficient flap area with autoarteria by continuous suture.;EFFECT: method ensures preservation of integrity of fibrous ring and proper leaflets of pulmonary arterial valve, mobility of leaflets and blocking function of pulmonary arterial valve after radical correction of tetralogy of Fallot.;1 cl, 1 ex
