Human Error Caused Accidents: Addressing a Critical Problem. Proceeding of the 26th Annual Workshop on Human Factors in Transportation and TRB Summary Sessions 1 and 40

These proceedings document activities from both the 26th Annual Workshop on Human Factors in Transportation and the sessions numbered 1 and 40 of the 1993 annual meetings of the Transportation Research Board. Both activities were coordinated by the Human Factors Workshop Planning Committee. The proceedings are not meant to be a comprehensive and definitive collection of actual occurrences. They were developed from materials provided either by the presenters themselves, or from summaries abstracted from the presentations. While the materials are thought to represent the activities, they have not been verified in their final form by the principals involved. It is hoped that the information provided will prove useful in further efforts toward addressing human factors issues-problems in transportation.
