This report was developed under contract No. 282-91-0074 by the Center forClinical Quality Evaluation (CCQE) for the Agency for Health Care Policy and Research (AHCPR). It contains four appendices on: Benign Prostatic Hyperplasia (BPH) pre-intervention materials, educational intervention materials, patient survey materials, and physician survey materials. It also contains a project design overview, abbreviations used in BPH criteria and algorithms, criteria lists and narratives, flowchart algorithms, criteria grid work sheets, computerized algorithm development and programs, codebooks, data collection work sheets, sampling matrices and sampling process, case exclusions, and inter-rater reliability evaluations.
