On June 5, 1998, a 26-year-old furniture warehouse worker was fatally injured after falling 18 feet from an order-picker forklift truck. The incident occurred as the victim was moving the raised forklift truck down a narrow aisle between the warehouse racks. No one saw the incident, but apparently the forklift platform (a flat cart designed to be carried on the forks) struck the metal warehouse racks, pulling the platform off the forks. The victim then fell from the cab of the forklift, striking his head on the concrete floor. He was placed on life support and died three days later. NJ FACE investigators concluded that, to prevent similar incidents in the future, these safety guidelines should be followed: (1) High-rider forklift operators must always use fall protection. (2) Employers should ensure that forklift operators are properly trained and certified as per the OSHA regulation 29 CFR 1910.178.
