The Istituto Superiore di Sanita (Italian National Institute of Health) implemented a study to detect maternal deaths and severe acute maternal morbidity, analyze associated causes and compute maternal mortality ratios (MMR) and severe maternal morbidity rates (SMMR) in six Regions of northern, central and southern Italy. The total MMR is 11,8/100.000 live births. This study finds a 63% under-reporting of maternal mortality based on current death certification alone. Haemorrhage and hypertensive disorders of pregnancy are the leading risk factors of obstetric deaths and near misses. The specific MMRs and SMMRs were higher for women older than 35 years, for women who underwent Caesarean section, for migrant women and for those with lower educational level. This study provides the background for a prospective study on registration and evaluation of maternal mortality to identify risk factors and contribute to improving the quality of obstetric care.
