Nd: YAG Laser for Posterior Capsulotomies

At present, extracapsular cataract extraction (ECCE) with intraocular lens implantation accounts for more than two thirds of all cataract surgery. Progressive clouding of the posterior capsule is a major adverse consequence of this technique and requires surgical intervention. Use of the Nd:YAG laser to photodisrupt the opacified posterior capsule has been shown to be a highly effective alternative technique to invasive surgery. This noninvasive technique is safer and at the least is equally as effective in opening the posterior capsule as is surgical invasion of the eye. The studies reported in the literature appear to demonstrate and support the safety and clinical effectiveness of Nd:YAG laser posterior capsulotomies. The only complication of any note has been the development of a temporary rise in intraocular pressure (IOP) in some patients. The American Academy of Ophthalmology has stated that the Nd:YAG laser appears to be safe and clinically effective and the treatment of choice for posterior capsulotomy. In the elderly, a noninvasive outpatient or office procedure would be especially useful. This technique should be performed by persons experienced and trained in Nd:YAG laser posterior capsulotomy.
