This Preliminary Hazard Analysis (PHA) of the BIPS-rs is a tabular summary of hazards and undesired events which may lead to system damage or failure of personnel injury or death. The PHA uses the following symbols to identify the "Program Phase" of interest:nGND - Ground Handling/TransportationnPAD - Prelaunch and CountdownnLIO - Launch and Insertion into Orbitn0P0 - Operational Use (any time DIPS is operating)
