There is no doubt that the oil and gas sector has taken operational safety much more seriously since the Piper Alpha disaster 25 years ago. The UK was at the forefront of sweeping regulatory reforms at the time, with Lord Cullen's official Public Inquiry outlining 106 recommendations for the assessment of risk management. Despite these measures in the UK, and around the world, 2,170 people still died between 1970 and 2000 as a result of major offshore incidents internationally. It is evident there is still room for improvement. In order to encourage a positive organisational safety culture, oil and gas industry leaders must focus as sharply on human behaviour as they do on technical risk. Effective risk management in the oil and gas industry must go beyond regulation and instead underpin a far broader organisational culture in which safety is integral to every aspect of a company's operation-however small that part may be.
