The article by Dr Shields titled "Medical Management of Carotid Stenosis" highlights the medical management options and effectiveness for the prevention of stroke. The strength of the article is that it documents very nicely optimal medical management both from the standpoint of preventing stroke and also reducing cardiovascular morbidity and mortality.The author asserts in his conclusion that with continued advancements in medical management, patient selection recommendations should become more limited. Is this a valid conclusion in the absence of randomized trials comparing optimal medical therapy with carotid revascularization?
