Unknown case: Low back pain in a patient with cafe au lait spots.

A 30-year-old man presented with chronic intermittent axial low back pain. The patient was managed with physical therapy and anti-inflammatory medications without relief. On physical examination, the patient had 5/5 strength in the lower extremities. Straight leg raise was negative, and lower extremity deep tendon reflexes were 2+ bilaterally. The patient was incidentally noted to have multiple cafe' au lait spots on his back and lower extremities. Plain films and magnetic resonance imaging of the lumbosacral spine were obtained.
