I read with great interest the article titled "Vitrectomy without face-down posturing for idiopathic macular holes" presented by Nadal et al.1 The authors claimed no need of the standard postoperative posturing technique, based on an anatomical and functional success rate of 87.3% for macular hole (MH) treatment with vitrectomy, internal limiting membrane peeling, gas tamponade, and combined cataract surgery without postoperative facedown posturing. They recommended this combined surgery because a second surgery for cataract extraction increased the risk of reopening the hole.
