We read with interest the article by Tosi et al, as it raises a number of important questions. First, it emphasizes the possibility of flap displacement during retinal surgery, even 24 months after laser-assisted in situ keratomileusis (LASIK). Similar situations have also been described by other authors. We agree that this possibility should be considered when operating on these patients, and thus, the measures required to manage this rare, but reported, complication. When dealing with intraoperative flap displacement during vitrectomy, we suggest considering, once the flap is repositioned, the placement of a soft piano contact lens over the cornea in a similar manner to the one described by us in a brief report published in RETINA, as a means of avoiding new dislodgements of the corneal flap when continuing with retinal surgery. In our report we suggested suturing a contact lens for scleral buckling procedures, as we did not know flap displacement occurred during vitrectomy. Recently, we have tried placing a contact lens over the cornea with sutured lens ring before performing vitrectomy surgery (20 and 25 gauge). The pressure exerted by the sutured lens ring helps to keep the contact lens in place and thus, the procedure of fixating the contact lens is simplified because there is no need for the continuous fixating suture that we performed in the case of scleral buckling procedures.
