An exploration of the association between male body mass index and semen quality

We read with interest the article of Shayeb et al. which reports an association between male body mass index (BMI) and semen quality (Shayeb et al., 2011). Using logistic regression the authors imply obese men are more likely to have lower semen volumes and higher abnormal sperm morphologies than men with normal BMI.There are several concerns and omissions about this communication that require further discussion. Conditions that may have an impact on semen quality (e.g. diabetes), need further evaluation in this population. Diabetes is a serious health condition often associated with obesity. Evidence from several studies suggests that obesity and weight gain are related to an increased risk of diabetes (Ford et al., 1997; Resnick et al., 2000). Recent findings have shown that diabetes induces subtle molecular changes that influence sperm quality and function (Mallidis et al., 2011).
