Krishnamoorthy for his insightful comments about our article on left atrial (LA) thrombus in rheumatic mitral stenosis in sinus rhythm.1 As a further additional point of detail, one of the primary objectives of our study was to estimate the LA appendage filling and emptying velocities in sinus rhythm patients with and without thrombus and to identify a cut off value for appendage velocities below which the risk of thrombus formation is statistically significant. As noted in our study the LA appendage filling (22.2 ± 12.6 vs. 24.3 ± 9.8) and emptying velocities (1 7.9 ± 10.5 vs. 20.6 ± 10.6) were low in both patients with and without thrombus. Therefore a reduced appendage velocity per se does not predict the risk of clot formation.
