Corrosion in pipes: the hidden danger

Acargo ship is lost and four crewmen die after a seawater pipe in the engine room bursts. The engine stops and the ship is blown onto a lee shore, where it breaks up. Why have the pipes been left to corrode to such an dangerous extent, without anybody noticing?
