Can we propose the 4Ds along with the 6Cs for modern British nursing in the care of older patients? An evaluative study shows the effect training can have on nursing care

Training and education is an important aspect to care delivery in the health sector. The Chief Nursing Officer of England, Jane Cummings, has proposed that compassion be at the centre of caring for the sick patient in the health care system. Compassion is an important value, but it is difficult to make a difference when applied in isolation. Commitment and competence have to be instilled in the staff for clinical excellence to prevail. The audit commission has reported that up to 70% of hospital beds are occupied by the older patients, and this trend is likely to continue (Audit commission, 2006). Accordingly to the UK hospital Episode Statistics, hospital stay for over 60s has increased by up to 66%. Dementia, delirium and depression make up the majority of the mental health comorbidities presenting to the acute hospitals.
