Hip dislocation and femoral neck fracture: decision-making for head preservation.

Hip dislocation with femoral neck fracture is a rare injury in young adults and is a challenging problem. It is fraught with potentially devastating consequences including avascular necrosis and subsequent early secondary osteoarthritis.We identified only 30 cases reported in literature that bear this fracture pattern (Table 1). Of note 17/30 cases were treated by open reduction and internal fixation and 9 of these 17 have developed avascular necrosis leading to eventual prosthetic replacement. Based on this evidence in literature many authors have even proposed primary hemi- or total hip arthroplastv.
