Cleft mitral valve.

A 30-year-old woman with New York Heart Association class III dyspnoea, a pansystolic murmur and severe mitral regurgitation on echocardiography was referred to our unit. Two-dimensional trans-thoracic and trans-oesophageal images raised the suspicion of cleft mitral valve as (1) the regurgitant jet appeared to originate through the anterior mitral valve leaflet and (2) there appeared to be a division in the anterior mitral valve leaflet. The left ventricle was hyperdynamic and not dilated, there was normal offsetting of the AV valves. Mild aortic regurgitation was also noted. We performed real-time three-dimensional echocardiography (IE33, Philips) using a 37 Hz probe, acquiring a full-volume image panel A. This clearly identified a cleft in the anterior mitral valve leaflet, confirming the diagnosis. Our patient was subsequently referred to the regional congenital unit for surgical repair of the cleft mitral valve.
